Our Community Support service enables adults with life-limiting conditions, their carers and families, to adapt, manage and understand their diagnosis in a holistic manner in their own home.

This personalised service may provide light-touch signposting and guidance for future planning to a family, but it might also include liaising with other organisations to support adjustments or to facilitate smoother and more timely transitions between our services, or those provided by the wider clinical/social care community.
